Our practice catchment area helps us ensure that patients can access safe, timely and effective care from our team. By registering patients who live within a defined geographical area, we can manage demand appropriately, maintain continuity of care and provide essential services, including home visits where clinically necessary.
If you're considering registering with Bradley's Practice, please check that your home address falls within our catchment area before completing your registration.
Once you've confirmed that your address falls within our practice catchment area, you'll need to complete a Registration Form (GMS1) and a New Patient Questionnaire. Both forms are available from reception at either of our surgeries. Alternatively, you can download them below and complete them at home before visiting the practice. Each person registering must complete their own set of forms. This helps our team understand your healthcare needs while we wait for your medical records to be transferred. When registering, please have your NHS number, proof of address and photographic identification ready. A list of accepted identification documents can be found in the New Patient Questionnaire.
