New Patient Registration

How to register with our practice

Before registering with our practice please check our area map first to ensure that you are within the area that we cover. This is decided by the Local health Board.

Registration Forms

Once you have checked the area map, complete a Registration Form (GMS1) and a New Patient Questionnaire. You can get these from reception at either of our surgeries, or to save you travelling to the practice you can download them below.

We’ll need one from each person who is registering so that we can learn a little about you before your medical records arrive with us.  You will need your NHS number, proof of address and photo identification (acceptable forms of ID are listed on the New Patient Questionnaire).

*Please note new born babies should be issued with an NHS number before they are discharged from hospital.

Standard of Service and Responsibilities of Patients

You are entitled to receive a high quality of care which we always strive for, and wish to be informed if we do not live up to. In return, we request that patients attend their booked appointments and cancel/reschedule if they are unable to do so (we use Zymego which makes this quick and easy; see our Appointments page). Patients who repeatedly fail to attend appointments risk their future registration status being reviewed.
 
As a patient you can expect your medical details to be shared with other healthcare professionals if necessary, in accordance with our Fair Processing Notice.
 
The practice demands that all our staff are treated with dignity and respect as they go about their work. The practice will deregister patients if an act of violence against a member of the practice team occurs.  The practice adopts a Zero-tolerance policy and will inform the police if necessary.

Please note no form of abuse including Verbal abuse will be tolerated.
