New Patient Registration

Before registering with our practice please check our area map first to ensure that you are within the area that we cover. This is decided by the Local health Board.

Registration Forms

Once you have checked the area map, complete a Registration Form (GMS1) and a New Patient Questionnaire. You can get these from reception at either of our surgeries, or to save you travelling to the practice you can download them below.

We’ll need one from each person who is registering so that we can learn a little about you before your medical records arrive with us.  You will need your NHS number, proof of address and photo identification. 

* Please note new born babies should be issued with an NHS number before they are discharged from hospital.

Once you have completed the forms you will be invited for a new patient check. For children under 5 a child health surveillance form must be completed for registration purposes. When registering you will be asked to provide proof of identity in line with the local NHS policy (acceptable forms of ID are listed on the New Patient Questionnaire)

Standard of Service and Responsibilities of Patients

You are entitled to receive a high quality of care, but at the same time we do expect our patients to keep their appointments. Patients who repeatedly non-attend appointments may receive a letter from the practice outlining that their future registration status will be reviewed if further failed encounters occur.
 
The patient can expect his/her medical condition to be shared with other health care professionals if felt necessary, in accordance with our Fair Processing Notice.
 
The practice will remove patients from its list with immediate effect, if an act of violence against a member of the practice team occurs or if that member of staff fears for their personal safety.  The practice adopts a Zero-tolerance policy and will inform the police if necessary.

*Please note no form of abuse including Verbal abuse will be tolerated.
