Before registering with our practice please check our area map first to ensure that you are within the area that we cover. This is decided by the Local health Board.
Once you have checked the area map, complete a Registration Form (GMS1) and a New Patient Questionnaire. You can get these from reception at either of our surgeries, or to save you travelling to the practice you can download them below.
We’ll need one from each person who is registering so that we can learn a little about you before your medical records arrive with us. You will need your NHS number, proof of address and photo identification.
* Please note new born babies should be issued with an NHS number before they are discharged from hospital.
Once you have completed the forms you will be invited for a new patient check. For children under 5 a child health surveillance form must be completed for registration purposes. When registering you will be asked to provide proof of identity in line with the local NHS policy (acceptable forms of ID are listed on the New Patient Questionnaire)
You are entitled to receive a high quality of care, but at the same time we do expect our patients to keep their appointments. Patients who repeatedly non-attend appointments may receive a letter from the practice outlining that their future registration status will be reviewed if further failed encounters occur.
The patient can expect his/her medical condition to be shared with other health care professionals if felt necessary, in accordance with our Fair Processing Notice.
The practice will remove patients from its list with immediate effect, if an act of violence against a member of the practice team occurs or if that member of staff fears for their personal safety. The practice adopts a Zero-tolerance policy and will inform the police if necessary.
*Please note no form of abuse including Verbal abuse will be tolerated.

We have a service enabling our patients to access a GP Consultation Online, this is provided via eConsult and is available by accessing the link below. You can use this service at a time when it is convenient to you, by using your regular online device. The service enables you to ask for advice from your GP, research self-help information or get in touch with the practice if you have an administrative request.
With an eConsult online consultation, you might be asked to upload photographs of the affected area. Please ensure that these photographs are of excellent quality and clarity, so they can be assessed properly. It is NEVER appropriate to send photographs of sensitive parts of the body such as genitals. If these are submitted they will be deleted and not be added to your medical record. This also applies to photographs of children e.g. a photograph sent in to show nappy rash, would not be acceptable.
This service is available from 06:00 - 17:30 each working day - Monday to Friday - excluding Bank Holidays.
If you are in need of medical attention outside of these hours please call NHS 111 or attend A+E if it is an emergency.
Book an Online Appointment via eConsult
Our appointment system has recently changed, we are now able to offer an urgent on the day appointment if necessary and also a routine appointment service.
If we experience excessive demand during holiday periods, we have to prioritise urgent/emergency requests on the day.
For any administration queries, please refer to the other sections on this website for alternative ways to contact us in order to relieve pressure on the telephone lines.
If you require a medical appointment please use eConsult our online appointment service (see above). However if you are unable to use eConsult you can phone or call into the practice for a medical appointment. We will take your details and ask you a number of important questions, which will ensure that your request can be triaged correctly.
You will then be added to our Care Navigation Appointment system, where our Navigation team will assess your request and you will be contacted by either phone, email or text message. (Please be aware that you may be invited in for a face to face appointment that day).
For your information, a GP leads the navigation team every day.

If you have contacted us via eConsult, then we will email our response to you via the eConsult link – so please check you emails(and spam) before contacting us to say that you haven’t heard from us.

Routine appointments are available up to six weeks in advance, if you request an appoinment via eConsult or telephone, you will be assessed and either an on the day appointment or routine appointment will be offered if necessary.
We are lucky to have 9 GP partners here at the surgery. Although unfortunately it isn’t always possible to see the doctor of your choice as almost all of our GP’s are part time. Annual Leave also affects this wait and covering as duty doctor for other doctors that are away or on Courses /Sick Leave etc.
Please note: If you or a member of your family are ill, you will be seen more quickly by making an appointment at the surgery. The doctor can see several patients in the surgery in the time that it takes to make one house call and will have better equipment and facilities to provide treatment where necessary. Transport problems are not usually an acceptable reason for requesting a home visits.
Home visits are available if you are housebound and are unable to get to the surgery. Please consider the following guidance when requesting home visits:

When the practice is closed, Out of Hours Service NHS 111 is available:
Weekdays: 18:30 - 08:00 hrs
All Day: Saturday, Sunday and Public Holidays.

You may prefer to have someone else in the room with you if the doctor or nurse needs to examine you, particularly if the examination is an 'intimate 'one.
The surgery is able to offer chaperones in this situation if you wish. These chaperones will either be other clinical staff (doctors or nurses) or trained receptionists. All staff are bound by strict confidentiality guidelines. If you would prefer to have a chaperone present please let the receptionist know at the time of booking your appointment if at all possible. Occasionally it may be necessary to re-book your appointment if you wish to have a chaperone present, if we are unable to provide one at the time.
We have introduced a text messaging reminder service, this system reminds patients of all face to face appointments with both Doctors and Nurses as well as our Health Care Assistants, Physiotherapy and Audiology clinics that are available.

We are proud to be a training practice. There may be students in an appointment with a GP or nurse that you are seeing, or you may see a GPs in training who is under supervision from one of our GP partners. Please let reception know if you are not comfortable with this.
Written consent is required if you wish to speak to our staff regarding someone else, such as a family member. Children aged 16 and over will need to provide consent for medical information to be shared with their parents. Please contact the practice by submitting a Contact us enquiry form.
We kindly request that all patients who no longer require their booked appointments let us know. This will enable other patients to attend. Patients who fail to cancel their unwanted appointments are contributing to a waste of resources.
To cancel an appointment - if you are registered on My Health Online, this can be easily done by logging in and accessing your appointment section.
Otherwise, please contact us as soon as possible.
The Practice adheres to a Did not Attend policy which supports the removal of a patient from its list if they repeatedly fail to attend pre- arranged appointments.
