We are looking for a dynamic and organised individual to join our team here at Bradleys Practice to summarise medical records. Previous experience of working within a GP Practice would be beneficial but not essential and knowledge of the Vision / Emis clinical system is desirable, however training will be given.  You will be required to summarise and read code relevant information from patients medical records onto the practice clinical system (currently Vision), working in accordance with the practice notes summarising protocol. Basic knowledge of medical terminology would also be an advantage. The Freedom of Information Act 2000 (FOIA) is designed to increase transparency; providing members of the public the opportunity to routinely access information held by public authorities, such as the Practice, subject to certain exemptions. It does this in two ways:

  • public authorities are obliged to routinely publish certain information about their activities; and
  • members of the public are entitled to request information from those public authorities.

In accordance with the FOIA, the Practice has produced a Publication Scheme

Requests for information under the FOIA must be made in writing, for example, in a letter or an email, providing your name and either a postal or email address. The Practice has 20 working days to respond to your request.

Publication Scheme

Class 1 – Who we are and what we do

Practice details, information about our Practice team and our opening times are all published on our website. This information can be accessed via the About Us page. 

Class 2 – What we spend and how we spend it

The Practice receives money from NHS Wales according to its contract for national General Medical Services in exchange for services provided for patients.

This information can be obtained on written request to the Practice Manager by writing to Bradley’s Practice, Alltami Road, Buckley, Flintshire, CH7 3PG or by hand delivering a written request to our Reception Team. Alternatively, please contact our Reception Team on 0345 900 7851  if you have any queries. 

There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes, under the Act, has taken the view that it may be prejudicial to the conduct of the Practice’s affairs.

If this is the case, we will respond to your FOI request with a formal letter acknowledging the reasons why we are unable to give you this information.

Class 3 – What our priorities are and how we are doing

  • Working closely with our colleagues in the South Flintshire Cluster to develop and improve services for patients in the local area. 
  • Working towards achieving required standards such as Quality Assurance Framework (QAIF), Access Standards, Medicines Management, PatientSafety, IG toolkit and Clinical Governance Practice Self-Assessment Tool (CGPSAT). 
  • Working with our neighbouring practice and the Health Board to continuously improve the services that are offered within Buckley Medical Centre and our Mold premises at New Street Mold CH7 1PA
  • including facilities improvement and ongoing maintenance of the building. 
  • The practice have not had a recent inspection from Health Inspectorate Wales (HIW). When we do have an inspection, the report will be published here. 
  • The practice was previously working together with our Patient Participation Group (PPG) to improve the services that are offered by the practice.  These  meetings were put on hold at the start of the pandemic but we are hoping to  restart them as soon as we can. 

Class 4 – How we make decisions

Management policies are formulated at regular business meetings. Decisions affecting the provision of NHS services are recorded. These are available on written request (any information which is commercially sensitive or falls under the Data Protection Act is excluded).

  • Policies and procedures regarding employment of staff 
  • Zero Tolerance Policy
